75 year-old with recent constipation, 4 days of increasing abdominal distention, rare small loose bowel movements, and an abdominal xray gas pattern suggestive of possible distal colonic obstruction. Sigmoidoscopy revealed a contracted segment shown here, with a suggestion of a counter-clockwise twist, above which the colon was markedly dilated.
